Upon identifying a participant’s needs, strengths, and primary problems, they’ll work together with clinicians to develop an individualized plan of treatment. Length of stay ultimately depends on progress - the Transition’s website states that original discharge dates are “never set in stone” - participants of this long-term program can reside here <strong>from nine months to a year</strong> (some stay even longer) before continuing with intensive outpatient (IOP) or another level of outpatient care.</p> <p>Like other Transitions programs, The Grateful Life Center provides <strong>services that are shaped by a handful of specific goals</strong>.
